    • School Physical
    • Blood Lead Screening
    • Up to Date Immunizations

     

    Kindergarten

    • School Physical
    • Blood Lead Screening
    • Up to Date Immunizations, Including 2nd MMR and Varicella and dTap and IPV boosters
    • Dental Exam (by May 15th of student's Kindergarten year)
    • Eye Exam (by October 15th of student's Kindergarten year)

     

    2nd Grade

    • Dental Exam (by May 15th of student's 2nd grade year)

     

    6th Grade

    • School Physical (sports physical will NOT be accepted)
    • Up to Date immunizations, Including 2nd MMR and Varicella, 1 dose of Menactra and a Tdap vaccine
    • Dental Exam (by May 15th of student's 6th grade year)
