and Instructions for Nominator
Please read the following information carefully before submitting a nomination:
I. The Award
The Distinguished Alumni Award shall be given annually to recognize past graduates of Hillsboro High School, Coffeen High School, and Witt High School.
II. Eligibility
The Distinguished Alumni Award shall be given annually to recognize past graduates of Hillsboro High School, Coffeen High School, and Witt High School. Awards for alumni may be given for achievement in various fields of service which may include, but are not limited to, the arts, athletics, business, education, entertainment, law, medicine, military, music, philanthropy, politics, public service, religion, scholarship, science, or any other significant area of achievement.
III. Nomination Procedure
A. Any individual may submit a nomination.
B. Download and save the Nomination
Form to your computer. Complete the nomination form.
The nominator may
attach documents to this form.
C. The Awards Committee shall consider all nomination information confidential.
D. Nominations will open on December 1, 2008 and must be received by February 28, 2009 for the 2009 Annual Award.
E. Return the completed Nomination Form to:

Below are some suggestions as to information you may wish to include in or with your written nomination.
Career Summary: Positions held, employers, business accomplishments;
professional practices. You may wish to include any military service here.
Civic, Philanthropic, Religious, Fraternal and Political Activities:
Offices held, accomplishments, record of service.
Professional or Commercial Activities: Corporate directorships
or such similar activities, teaching, consulting, professional organizations
or civic organizations.
Non-Business Directorships and Trusteeships: Service on Foundations,
Boards, Trusts, Not-For-Profits, hospitals, etc.
Citations and Awards: Business awards, professional recognition,
public service awards, military decorations, etc.
Noteworthy or Special Work: Research, inventions, publications,
outstanding artistic or professional works, etc.
Attachments:
In addition to the nomination form, you may submit other information for the selection committee's consideration, which will demonstrate the qualifications of this individual. This may include a vita, newspaper or magazine clippings, etc. Please be as concise as possible in order to facilitate the sharing of this information with the committee.
